If I print my labels from eBay to PayPal and PayPal adds the insurance to the cost, has that been purchased through Auctiva or am I paying for it twice?
I also print my shipping labels through PayPal. The big questions are: 1) are you using ebay or Auctiva checkout, and 2) if you're using Auctiva checkout, are you setup using Auctiva insurance.
I use ebay checkout and Auctiva insurance. I don't buy the USPS (or other) when I print the shipping label and manually make the purchase of the U-Pic insurance at Auctiva. I have to do it manually because only Auctiva checkout makes the purchase of their insurance automatically.
The insurance purchased at the point where you are creating a shipping label is through the carrier, e.g. USPS Insurance. When you make the label, there is an itemized amount covering the value of the package (should be filled-in as ebay final value). If that's checked off, you're buying carrier's insurance (not Auctiva).
Now, if you're seeing a wrapped cost for Shipping and Handling that includes insurance and using Auctiva Checkout, your Insurance is purchase automatically (if your settings are correct), and should see the purchases in your transaction log. When the customer does PayPal through Auctiva checkout, the Insurance isn't itemized (from what I've been told by Jeff the CEO and others) in the PayPal transaction.
I hope that helps. Or did I just confuse you more?

I am totally confused. All I want to know is when a buyer pays for insurance with Auctiva and I print a USPS label through Paypal, do I have to check the insurance box, or has it already been paid for through Auctiva? I don't want to pay twice.

Since there's insufficent information for your specific case, here's the best answers I can give.
A yes or no answer would be perfect.
If you use Auctiva checkout and are using Auctiva insurance.
No
If you use Auctiva checkout and are not using Auctiva insurance.
Yes
If you use ebay checkout and are using Auctiva insurance.
No
If you use ebay checkout and are not using Auctiva insurance.

If your buyer purchases insurance through the Auctiva Checkout system and Auctiva automatically generates an insurance policy to cover that item, then you should NOT purchase additional shipping insurance when printing your shipping labels through Paypal. If you were to do so, you would effectively be purchasing insurance twice for the same shipment.

I'm confused on how the Buyer is optioning for Insurance at checkout, which adds to total; and then is paying a lower amount then total. Or does "never pays" mean no payment was made period? Or does the optioning of insurance not add the Insurance cost to total?
Can you give more specifics, e.g. PayPal payment as method, a listing # for problem transaction, using eBay or Auctiva checkout?
