Sorry, it won't work. The only way eBay can tell you want your listing to be handled by a Third Party Checkout provider is if that value is attached to the listing at listing time, and there is no way to do this using the SYI form on eBay. Why do you think the only way your custom template will work is if you use eBay to list it? Why can't you copy the same html into your description on Auctiva?

Yes, you can manually add Auctiva Shipping Insurance to your listings by going to http://www.auctiva.com/sales/orderstransactions.aspx (or click on SALES > TRANSACTIONS in the navigation tabs). If you ship multiple items to the same customer, you can also use combined policies. Right now the only way for that to be done automatically is to use Auctiva Checkout, but you can file a support case to have a combined policy created for you by our Customer Support department.
