I can help you with your first question:
Go to My Account>eBaySettings>Update Store Categories from eBay. This will import your current Store Categories from eBay. You will have to do this every time you change or create a new category on eBay.
As for the template, post your question in the Templates & Custom Templates section. There are a lot of helpful members who will be able to help you. Or file a Support Request ticket with the link enclosed.
