Hi,

No mystery on how this one became FREE to the buyer. The Insurance is clearly marked "Included in S&H" in the listing, i.e. the Insurance was part of FREE.

Question is: Was that what was selected in the one-page lister? You need to check the Saved Listing for how you have Insurance selected. If you didn't select it that way, I'd start a Support Case with Auctiva for them to investigate why it changed on submission to eBay.
