The practicewriter shows both a [DESCRIPTION] and [DESCRIPTION_AND_IMAGES] tag present in the code (but not on the visual). Also, the later is actually inside a comment field, so it actually won't work correctly. Comments start with . I'd remove the later and change the former to the D&I tag.

I also see an extra [IMAGE1] tag in a comment field. That's why a visual check doesn't show them. It looks like template maker comments to fill-in info is were some of the problems are occurring.
