Are you using Calculated or Flat Rates, as I think only Flat has that "Additional Item Cost" box?

If you're using ebay combined shipping rules for Calculated and those aren't being applied, have you tried refreshing that in the Auctiva account settings for ebay. That page has an "Update Combined Shipping Discounts From eBay" link to accomplish that function. For those to work, you also have to check the "Apply my eBay combined shipping rule" on the Lister.
